Vive Bien/Live Well is a free mobile application ("App") developed with New Mexico seniors in mind to help stay healthy, independent, and strong.
Seniors can do "Health Checks" in 6 key areas that shape seniors' health (Memory, Mobility, Nutrition, Mood, Vision, and Hearing).
The App includes a simple set of questions, and provides prevention tips and next steps in a summary called "Care Plan".
Health Checks are recommended every 6 months because this is not a one-time check; it is a companion tool for healthy aging.
New Mexico is the first state in the U.S. to bring a program like this to its communities, making it easier for older adults to get the support they need to age well.
New Mexico has a large aging population, with seniors experiencing high poverty level in a large rural that limits access to care.
Sometimes it’s hard to get the help we need- maybe because we live far away from a doctor, don’t have a ride, or are busy at home. That’s why the Vive Bien Live Well program is here.
We want to help seniors in New Mexico stay healthy and live independently.
Vive Bien/Live Well seeks to empower New Mexico's seniors through person-centered care, enhancing health, independence, and quality of life by improving services, resources, and community support for healthy aging.
Vive Bien/Live Well helps seniors health in 4 easy steps.
First, you answer a simple set of questions to check how you're doing in areas like memory, movement, eating, and more.
Second, based on your results, the App shares the next steps, including prevention tips, recommendations, and local resources.
Third, you get a care plan tailored to you, with a summary of your health checks and resources. You can share it with your health care team or family.
Finally, you can follow up and recheck in 6 months or sooner if you'd like.
Vive Bien/Live Well is funded by the New Mexico Health Care Authority and organized by the University of New Mexico (UNM) with contributions from managed care companies, healthcare professionals, community partners, state entities, and a Community Advisory Council that includes seniors themselves.

When something changes in these areas, it helps catch it early, so you can get care right away.
It is ideal for seniors to be prepared before beginning health checks.
You would need up to 30 uninterrupted minutes per section to complete the steps in the app. Before starting, the app provides an overview of what is needed for the assessments.
If assistance is required, it is recommended that a caregiver or support person be present during the process to ensure it goes smoothly. It’s best to start when you feel well and have time to complete it without interruption. Don’t do it when you feel sick.
The App provides next-step guidance and local resources, including preventive strategies, information when check-ups are needed again, or a recommendation to seek clinical evaluation.
These are summarized in a personalized care plan that seniors can download and share with their healthcare provider to support informed decision-making and early intervention.
