Vive Bien/Live Well is a person-centered program designed to support seniors from New Mexico in maintaining their health, independence, and quality of life through evidence-based assessments and personalized care plans via a mobile app.
This program was adapted from the World Health Organization (WHO) Integrated Care for Older People (ICOPE) to help seniors stay healthy, independent, and strong as they get older.
New Mexico’s senior population is rapidly growing- by 2030, one in four residents will be 65 or older. As people age, physical and mental health needs often increase, and access to care can be limited by distance, transportation, or daily responsibilities.
The Vive Bien Live Well program supports healthy aging by identifying needs early, reinforcing existing strengths, and promoting independence.
It offers a proactive, community-based approach to help seniors maintain well-being and stay connected.
The Vive Bien/Live Well program has a mobile application available on Google Play and the Apple App Store that helps older adults assess their health in key areas such as cognition, mobility, nutrition, vision, hearing, and mental health.
Through guided assessments, it identifies potential concerns early, supporting timely intervention. Family members or caregivers can also assist with the evaluations, ensuring a more comprehensive understanding of the individual’s health status.
The app is HIPAA-compliant, ensuring the protection of patient information. This streamlined communication supports coordinated care and enhances clinical decision-making.
Vive Bien/Live Well in New Mexico is funded by the New Mexico Health Care Authority and organized by the University of New Mexico with contributions of New Mexico Managed Care Organizations, healthcare professionals, community partners, state entities, and a Community Advisory Council that includes seniors themselves.
Vive Bien/Live Well brings a completely free mobile application that can be downloaded and used by seniors in New Mexico aged 65 and older.
The application can be accessed on Apple and Android mobile devices via the respective app stores.

The Vive Bien/Live Well program is not intended to replace clinical care or medical advice. It serves as a patient-facing tool to help older adults better understand their health and stay engaged in preventive care.
If the app identifies potential concerns, it provides tailored suggestions and encourages users to share this information with their current healthcare provider. This can support more informed discussions during clinical visits and highlight issues that may otherwise go unaddressed.
Healthcare providers can receive Seniors’ Personalized Care Plans from the Vive Bien Live Well App. These Care Plans have a summary of health checks done and findings, and include tips and community resources for seniors.
Yes, the New Mexico Vive Bien/Live Well program was built using evidence-based tools that have been tested and proven to identify concerns in seniors' intrinsic capacities. All of the training materials are available on our website in the Providers section.
The Vive Bien/Live Well team checks aggregate results from the app, listens to how people feel about using the app, and reviews feedback. This helps us improve and make sure the program is doing a good job.
Seniors need to provide their full name, birthday, gender, language (English or Spanish), ZIP code, phone or email, and their Medicare or Medicaid numbers.
This helps protect your account and keep it unique.
User data is protected by law (HIPAA). The app uses modern security techniques and operates on fully de-identified data that remains encrypted and protected.
Program analysis is completed with completely de-identified data sets to maintain individual patient privacy and security.
Seniors can download and share a report of their health check results from the evaluations from Vive Bien/Live Well App anytime they desire.
It’s a great way to talk with healthcare providers or caregivers about current health checks.
If the App identifies a potential concern, seniors are prompted to complete a more detailed follow-up assessment to clarify it.
Based on provided responses, the app will guide users to next steps, which may include preventive strategies, self-care tips, or a recommendation to seek clinical evaluation.
These actions are summarized in a personalized care plan that patients can share with their healthcare provider to support informed decision-making and early intervention.
The app includes local resources from New Mexico organizations that offer help and support.
All data will be securely stored using cloud-based services that comply with the latest security and encryption standards. Individual data is not accessible by Vive Bien/Live Well team members.
It is ideal for patients to be prepared before beginning the assessment process. It’s recommended to visit the Vive Bien/Live Well website for instructions on how to register and use the mobile application.
If assistance is required, it is recommended that a caregiver or support person be present during that time to ensure the process goes smoothly.
